Valence Health and Emmi Partner to Improve Patient Outreach


Valence Health, a leading provider of clinical integration, population health technology and value-based care solutions, announced today that it has entered into a partnership with Emmi, provider of superior patient engagement solutions, to help provider organizations utilize cost-effective, scalable ways of engaging patients in their healthcare. Their jointly delivered outreach campaigns will both inform patients about the importance of preventive care and empower them to take action.

Physician's Health Services (PHS) of Dalton, Ga., an Independent Physician Association (IPA) and a 50 percent owner of Alliant Health Plans, will be the first provider organization to benefit from this collaboration.

The partnership between Valence Health and Emmi will initially leverage care gap identification and/or case management assignment lists generated by Valence Health's Vision™ and vCare™ population health technology solutions. These lists identify patients who have not received key healthcare services. Providers then employ Emmi's library of more than 50 different outreach campaigns to generate patient calls that can help simplify complicated medical information and manage expectations around procedures, chronic health conditions, the hospital experience and more. All patient outreach campaigns are personalized to reflect each healthcare organization's name and the patient's physician's name.
