Humana, AMC Health Activate Post-Discharge Telehealth Program for Medicare Members

Humana At Home, the company's national division that supports more than 900,000 people with chronic health conditions, will provide the 30-day, post-discharge Interactive Voice Response (IVR) program to 1,000 Medicare Advantage members in 35 states who have been recently discharged from the hospital and are at risk for readmission.

Participating Humana members will receive weekly IVR calls to ask them about symptoms, such as fever, swelling and shortness of breath; assess pain management and wound healing; address post-discharge needs, such as medications; and remind them about follow-up physician appointments. Importantly, members will have the option to request a call from the company's clinical support team.

"We know that members leaving the hospital are vulnerable to complications and, althouh we already have a number of successful programs to help our Humana members transition from hospital to home, we're always looking for effective ways to expand those efforts," said Humana At Home President Eric C. Under the partnership, AMC Health will deliver technology and clinical support services and help gather information regarding the member's health status, and serve as an additional resource to Humana At Home's current care management program. During this 30-day collaboration, AMC Health nurses will identify gaps in care, provide coaching and education when appropriate and arrange for an appointment with the member's physician if further intervention is needed.
